Worksite Learning Agreement Process

Description of the Process

The Worksite Learning Agreement process is designed to formalize the work-site learning experience through a series of steps involving the completion of various forms and obtaining necessary approvals. This procedure ensures that all parties involved, including the employer, student, instructor, and director, are aligned and informed throughout the work-site learning period.

Steps to Complete the Worksite Learning Agreement

Step 1. Open Work-Site Learning Agreement Form

Step 2. Fill Out the Form

  • Complete all fields in the form, excluding the Parents' information for adult students.

  • If unsure about your work-site learning coordinator, contact your director for assistance.

  • Click the submit button once all fields are filled.

Step 3. Email Confirmation and Signing Process

  • After submitting the form, an email with a signing link is sent to the employer, student/parent (for high school students), instructor, and director, in that order.

  • Each signature prompts a confirmation email to the instructor and work-site learning coordinator to track progress.

Step 4. Completion Confirmation

  • An Approval Report email is sent to all involved parties upon all signatures being collected.

Step 5. Course Objective Checklist Email

  • An email with a link to the Work-Site Learning Course Objective Checklist form is sent to the instructor.

Step 6. Complete the Course Objectives Checklist

  • Ensure alignment with the Plan of Study, detailing objectives covered.

  • Additional courses can be added by uploading a Word document.

  • Click submit upon completion.

Step 7. Employer Receives Week 1 Attendance Form

  • The employer receives a link to the Work-Site Learning Attendance Form for Week 1.

Step 8. Fill Out Week 1 Attendance

  • The employer fills out accurate dates, times, and performance ratings.

  • If it's the final week, select "Yes" for the final week prompt.

  • A copy is sent to the instructor and coordinator for records.

Step 9. Week 2's Attendance Form

  • Upon submission of Week 1's form, the employer receives Week 2's form link.

  • Fill out similarly to Week 1, ensuring accuracy.

Step 10. Completion Confirmation

  • The employer confirms the student's completion of their work-site learning on the final attendance form.

Step 11. Employer Receives Evaluation Form

  • Upon submitting the final attendance form, the employer receives a link to the Work-Site Learning Evaluation Form.

Step 12. Evaluation Review

  • The completed evaluation is sent to both the instructor and the work-site learning coordinator for review.
