Step 1: Review Approved Clinical Agreements

  • Review the list of currently approved clinical agreements scheduled for presentation at the board meeting on August 13, 2024. Deadline to have these updated is May 17, 2024.

Step 2: Verification and Updates

  • Verification

    • Confirm All Facilities: Ensure all clinical facilities associated with your campus are listed in the attached document.

    • Removing Facilities: If a facility needs to be removed, highlight the facility name and use the strike-through feature in Word to indicate the removal.

  • Updates

    • New Contracts: Add any new clinical rotation agreements under the "New Contracts" heading in the attached list. Follow the provided format below and ensure all details are accurate.

      • Contract Requirement: Each new contract listed must have a corresponding new agreement. Direction are below.

  • Accuracy Check: Verify the correct spelling and legal names of the facilities using the facility’s website or the Oklahoma Secretary of State’s website for business registrations.

Step 3: New or Outstanding Agreements

  • Adding Agreements: List any new clinical rotation agreements that need to be added under the "New Contracts" section.

  • Outstanding Agreements: Ensure all agreements not listed but should be included are followed up promptly.

  • Procedure: Utilize the HCE Clinical Rotation Agreement JotForm for both new and outstanding agreements. A Kiamichi Tech employee initiates the form, which is then sent to the Facility and Campus Director for signatures, before routing to Johnna Smith for final processing.

Step 4: Director's Acknowledgment

  • Action: At the bottom of each clinical site agreement letter, there is an acknowledgment that must be filled out.

 

 

This acknowledgment serves as a replacement for the previous process of initialing, scanning, and emailing the document. You no longer need to follow these steps.

Step 5: Ongoing Verification and Reporting

  • Periodic Reviews: Instructors are reminded to periodically review existing agreements to ensure that facility administration details are current. Agreements must be re-signed if there is a new administrator at a facility. Use form above.
